Mostly the questions asked in the first two weeks. A team that has integrated an EHR before knows to ask about HL7 versions and interface engines before scoping; a team that has not finds out in month three. The code is rarely the differentiator — the sequencing is.
Yes, and earlier is cheaper. We would rather have a security or compliance reviewer in the architecture conversation than hand them a finished system to assess. Audit evidence, access controls, and retention rules are easier to design in than to add.
Production data does not go into development environments. We work against de-identified or synthetic datasets, access is scoped per engineer, and every access path is logged. Where a business associate agreement or equivalent is required, it is signed before any access is granted.
